About This Job
Arc Health is seeking a seasoned Nurse Practitioner (or Physicians Assistant - pay based on MGMA median) with a passion for providing healthcare to ages 5-18 for a full-time permanent position in northern New Hampshire.
Position Details:
- 1/2 time providing care in local schools, 1/2 time in primary care office.
- 3 days providing care in local schools to school-aged children, 2 days per week in Groveton clinic providing care to adults and children.
Responsibilities:
- Will include care coordination with the PCP
- Preventative health exams
- Complete physician examinations
- Acute care appointments for new or existing health problems (in person or via-telehealth)
- Providing education on health-related topics
- Health education specific to diabetes and asthma
- BMI assessments, nutrition education and physical activity counseling
- Immunizations
- Medication management
Requirements:
- Provider must have minimum of 3-5 years in practice in outpatient primary care
Clinic Details:
- EMR is Meditech Expanse, Dragon dictation
- Supported by an RN Team leader and designated MA
- Grant-funding through the State of NH
- School-based grant services to increase access to health services and behavioral care for children in our catchment area.
